Instructions on how to fill out a phone order form on your computer

1. Before filling out the form be sure to know your product codes, prices, quantity, shipping quote and colors.
2. You can fill out the form on your computer using Adobe Acrobat and send it to us back by email. SIGNATURE IS REQUIRED TO PROCESS CREDIT CARD.
3. If you do not want to fill out the form on your computer you can either download the form, print it, fill out, and send back by email: trendywristbands@gmail.com
   or by fax 1-800-945-1991. SIGNATURE IS REQUIRED TO PROCESS CREDIT CARD.
4. Each form has a customer service fee of $10.00 non negotiable, if you are doing a print job a proof is included.
5. Phone orders take up to 24 hours to process, due to the high volume of orders your order will be processed in the sequence in which they are received.
6. Once we receive your order we will process your credit card. Remember Credit Cards must match with the billing address.
7. If your order has artwork, we will be sending you an email with your invoice paid and artwork for your approval. Remember the later you respond to the email for approval the later your order will be processed.
8. Printing Tyvek 1 color ink can print and ship same day depending on the time we received and process order. Tyvek full color print takes up to 72 hours to have them done. Vinyl and plastic takes 10 business days or more.
9. Choose your correct shipping method, for example if you choose ground shipping, orders will take 3-5 business days to get to you.
